We are seeking a dedicated and compassionate Case Manager to join our team. This position offers the flexibility to work remotely and requires regular home visits and travel within the region.
Responsibilities
- Conduct assessments and care planning for clients in their homes.
- Develop, implement, and monitor individualized care plans to meet client needs.
- Coordinate with healthcare providers, social workers, and other stakeholders.
- Maintain accurate records and documentation using Microsoft Word, Excel, and email.
- Travel to client locations within Greenville, Greenwood, SC, and surrounding areas.
Requirements
- A 4-year degree, with a preference for Nursing, Social Work, Psychology, or a related health field.
- A minimum of 2 years of experience in care planning, assessment, or case management.
- A valid driver's license and car insurance.
- Proficiency in Microsoft Word, Excel, and email.
- Strong interpersonal and communication skills.
- Ability to work independently and manage a flexible schedule.
- Comfort with home visits and regular travel.
