About Us

Connections to Care is dedicated to providing high-quality case management services to our clients in the Greenville and Greenwood areas. We are seeking a dedicated and compassionate Case Manager to join our team. This position offers the flexibility to work remotely and requires regular home visits and travel within the region.

Responsibilities

  • Conduct assessments and care planning for clients in their homes.
  • Develop, implement, and monitor individualized care plans to meet client needs.
  • Coordinate with healthcare providers, social workers, and other stakeholders.
  • Maintain accurate records and documentation using Microsoft Word, Excel, and email.
  • Travel to client locations within Greenville, Greenwood, SC, and surrounding areas.


Requirements

  • A 4-year degree, with a preference for Nursing, Social Work, Psychology, or a related health field.
  • A minimum of 2 years of experience in care planning, assessment, or case management.
  • A valid driver's license and car insurance.
  • Proficiency in Microsoft Word, Excel, and email.
  • Strong interpersonal and communication skills.
  • Ability to work independently and manage a flexible schedule.
  • Comfort with home visits and regular travel.
