Senior Care Management with Connections to Care

Once the initial assessment has been conducted, our Care Manager will create a customized care plan for the client and caregivers. This personalized care plan will encompass the identified necessary services, including the names of organizations, contact details, critical points of contact, offered services, and pricing (where applicable). The Care Plan will be discussed with the client and caregiver, either in a face-to-face meeting or via a video conference.

 

Our Care Managers can carry out any or all the services outlined in the Individualized Care Plan as directed by the client or their family. The Care Manager will be involved right from the initiation of services. He will continue to conduct monthly check-ins to ensure that the services consistently align with the evolving needs of the client and their family.

Please note that fees are associated with executing the Individualized Care Plan.
